Primary Care Physician Selection & Medical Records

If your insurance requires a Primary Care Provider (PCP),  it is important to have S Schweinshaupt listed as your PCP.  This should be listed on your insurance card.  If another provider is listed, you either call member services or go to the insurance web site to change your PCP.

To facilitate patient care, your medical records will be taken from Northwest Medical and transferred to Dr Schweinshaupt's new office.  Send us an e-mail at Transfer-my-records@family-Dr.com or you can fax the request to (813) 969-2399.  Include your name and the name of all family members that will transfer with you.

Walk-In visits welcome

Appointments are preferred, especially for physicals and well care, but when you are sick just come in.  You can always call to see if there is a best time.  Feel free to check on the wait time to see if you want to come back.  We try to see walk-ins within an hour, but this is not always possible.
